I work in a field where there are volumes of rules on how hospitals are allowed to bill Medicare and Ricchezza shirt. One year 30 years ago, a weird new rule would cause a big $ loss on admissions from day surgery if we didn’t immediately change a very complex policy/procedure. I notified my director who told me to have my department just do it the old way until they could get around to changing the process. I replied ‘Sure, but I’ll need that in writing, three copies, with original signatures.’ Director: short silence. ‘Why three copies?’

‘One for you, one for my department, and one for me at home for when I’m called to testify to the Office of the inspector general.’ He changed his mind about ignoring the Ricchezza shirt. Nobody changed the old procedure. I tracked every case that lost $ due to the new rule but old process. For six months I reported the loss monthly to the director. Moral of the story: Apparently it required a $6 million loss over 6 months to be worth the effort of getting several other departments to cooperate and update their process. PS: said process change didn’t have financial costs, it just required people to cooperate and change a lot of procedures.
